There are two types of Passports that U.S. citizens are eligible to apply for:
- Passport Book - Can be used for land, air, and sea travel
- Passport Card - Can be used for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only. Can not to be used for travel by air.
U.S. Citizens can have both a U.S. Passport Book and a U.S. Passport Card, if desired.
Applying for a U.S. Passport
To guarantee a smooth application process, please bring the following items with you when applying for a passport:
- Photo Identification
- Certified Copy of Birth Certificate
- Certificate of U.S. Naturalization or Certificate of Citizenship
- Expired Passport, if applicable
- Personal check or money order payment
NOTE: Hospital Birth Certificates and Baptismal Certificates are not acceptable.
How to Apply for a First Time Passport
Complete Form DS-11: Application for a U.S. Passport
Submit Proof of U.S. Citizenship
- Certified U.S. Birth Certificate
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship
Provide Proof of Identification
- Valid Driver's License (clipped MN Driver's License with the yellow receipt is acceptable)
- Previously issued U.S. Passport Book or Card
- Valid Foreign Passport (current)
- U.S Certificate of Naturalization
- Current U.S Military or Military Dependent ID Card
- Current Government Employee ID (city, state or federal)
Submit a photo meeting U.S. Department of State requirements.
- Photos can be taken at the City of Maplewood for an additional $10.00
Submit Payment
- Personal check or money order is required to the U.S Department of State
- Cash or Check is allowed to the City of Maplewood
- Credit Cards are not accepted
Minor Applications (Under 18 Years of Age)
All passport applicants must be present at the time of application, regardless of age. Visit the U.S Department of State website for more information on special circumstances:
How to Renew a Passport
If you meet the requirements below, you should renew your passport book and/or card to the National Processing Center through the US Postal Service.
Your most recent U.S. Passport:
- Is undamaged and can be submitted with your application
- Was issued when you were age 16 or older
- Was issued within the last 15 years
- Was isued in your current name or you can submit legal documentaton of your name change
If any of the above statements do not apply to you, you must apply in person and follow the procedures listed for "How to Apply for a First Time Passport".

Passport Book Fees
- Adult Passport Book (16 years or older): $110.00; Valid for 10 years
- Minor Passport Book Fee (under 16 years): $80.00; Valid for 5 years
- Expedited Service: $60.00 per passport application
- Passport Application Execution Fee: $25.00
Passport Card Fees
- Adult Passport Card (16 years or older): $30.00; Valid for 10 years
- Minor Passport Card (Persons under 16 years): $15.00; Valid for 5 years
- Expedited Service: $60.00 per passport application
- Passport Application Execution Fee: $25.00
